PLEASE READ! Volunteer Agreement, Release and Waiver of Liability
By initialing this waiver, I acknowledge that I am volunteering with Chemung County Habitat for Humanity, Habitat for Humanity International, Inc., and their affiliates, officers, employees, donors, and volunteers (the “Released Parties”). I understand that my activities may include construction, repair, loading, unloading, travel to and from sites, and other related tasks (“Activities”). I also acknowledge that these Activities involve inherent risks, including but not limited to illness, injury, property damage, or death, as well as potential exposure to COVID-19 and other illnesses. I voluntarily assume all risks and agree not to participate if I know I am ill or contagious.
In consideration of being allowed to volunteer, I release and forever discharge the Released Parties from any and all claims, liability, or damages arising out of my participation, except in cases of intentional or grossly negligent conduct. I understand that the Released Parties do not provide health, medical, disability, or other insurance for volunteers and that I am responsible for my own coverage. I further acknowledge that children under 16 are not permitted on active construction sites, and those between 16–18 may have limited participation as outlined by Habitat’s safety guidelines.
I consent to emergency medical treatment and necessary transportation in the event of injury or illness, and I release the Released Parties from any claims related to such care. I also grant the Released Parties permission to use photographs, video, or audio recordings of me (and any minor children volunteering with me) for promotional, educational, or other lawful purposes without compensation. I agree to maintain confidentiality of any personal or health information I encounter while volunteering.
This waiver is intended to be as broad and inclusive as permitted by law. If any portion is held invalid, the remainder shall continue in effect. I have read and understood this document, and by signing below I knowingly and voluntarily agree to all terms, binding my heirs, next of kin, and legal representatives.
